Do You Have an 11 year old Student Entering 6th Grade In the Fall? 

All entering 6th grade students must have a Tdap vaccine and the date reported to school before the first day of school, The student must be 11 years old to have this vaccine. Vaccinations may be reported as soon as students turn 11 once received from your provider, pharmacy, community health clinic or many grocery stores. Please advise the middle school as soon as this vaccine has been received in order to ease the transition to middle school. You may also contact your school nurse if your student turns 11 after the first day of school.  Washington State allows exemptions based on medical, personal and religious reasons. These are and obtained and signed by your provider, parent and sent to school.  Report immunizations to  imsimmunizations@issaquah.wednet.edu and please include students full name, birthdate and name and date of vaccine. You may also fax info to 425-837-6855.
